Work with Optimal Living

Thank you for your interest in Optimal Living!  In order to begin working, you need to present listed documents to the hiring manager or training director on or before orientation.  Even if you have completed orientation, you will not be allowed to work unless these documents are in your records.  Your criminal background check and Health Care Registry check will be completed prior to hire.

The Documents include the following:

·       Completed Application

·       Valid Driver’s License

·       Verification of Auto Insurance

·       Verification of Auto Registration

·       Social Security Card

·       Current TB Results

·       Copies of Relevant Training in particular CPR & First Aid

·       Verification of Education (Diploma, Transcript) 

It is necessary to update this information as needed. When your driver’s license and auto insurance expire, current documentation must be submitted upon the expiration of the information in your personnel file. In addition, the TB test must be updated yearly. Any changes in your driving, health or criminal status should be reported immediately to your supervisor before arriving at work. 

Any additional experience and certification as well as the pre-requisites for employment are reflected in your Privileging and Credentialing. The Hiring Manager will review your work experience and credentials. The Hiring Manager will assign you to perform tasks based on your background.  Do not perform tasks which you are not assigned to do. It is very important that you inform the Hiring Manager of any additional training and/or any strengths or needs that you recognize.

Your supervisor will meet with you at least monthly for clinical supervision. These supervisions will include discussions about service plans, clinical procedures, privileging and credentialing, incident reporting, etc. 

Each supervision will be documented.

It is the policy of Optimal Living to hire only those persons who are lawfully authorized to work in the United States. As a condition of employment, individuals hired at Optimal by are required to present. Proof of identity and of their legal eligibility to work in the United States

I hereby authorize without reservation, any party or agency to furnish the above-mentioned information for the suppose of obtaining background check. I further authorize ongoing procurement of the above-mentioned reports at any time during my employment (or contract).
